Our office accepts most insurance plans (such as MetLife, Anthem, Guardian, Employee Plans, AGA, Jefferson Pilot, Lincoln Financial, etc…) and we will file your claims for you. We are only in network with Delta Dental, PHP and Signature Care for Parkview employees. Most policies will allow you to go to any dentist of your choice, but they may not pay as much if you go out of network. It’s important for YOU to check with your insurance company to find out how your benefits will be affected by becoming a patient in our practice.
Insurance companies will pay what they consider usual and customary fees. You will be expected to pay whatever percentage your insurance will not pay at the time of service. For example, if your insurance company pays 80% for fillings, we ask that you pay your 20% the day you come in. If you are not sure how much that will be, we will be happy to figure the amount out for you. Please keep in mind that the amount you pay is only an estimated guess of what you are responsible for. If your insurance company doesn’t pay what we estimate they will, you will be billed for the difference. If they pay more than we estimated, we will refund you the difference.
If you do not have an insurance card, we just ask that you provide us with the following information:
Name of insurance company
Mailing address and phone number for your insurance company
Your unique ID #, if it isn’t your social security number
Your group number
We accept payment in the form of cash, checks, debit cards, Visa, MasterCard, and Discover.
Patients without insurance should be prepared to make their payment in full on the date of service. We offer discounts for payment in full. Patients under age 60 paying with cash or check will receive a 5% discount. Patients over the age of 60 will received a 10% discount with cash or check. All patients will receive a 3% discount with credit card or debit card payments. We offer financing with Care Credit and Personal Finance Company (PFC), for those who qualify.
If you will not be able to pay in full, arrangements must be made at least 3 business days prior to the scheduled appointment.
